1. Create a new 400x400 image with a white background. Set the Background Color to the following Gradient:

Settings used:

  • Gradient = Metal Steel
  • Angle = 0
  • Repeat = 1
  • Style = Radial
  • Invert = Unchecked
  • Horizontal and Vertical = 50

Use the Rectangle Tool to draw a square that's 345x345 (or close to that size.) Make sure Create On Vector and Anti-alias are check. Center the object by going to Objects | Align | Center in Canvas.

Press Ctrl+Shift+B to create a Selection from Vector Object, then Convert the layer to Raster. (Layers | Convert to Raster Layer)

2. Go to Effects | Texture Effects | Mosaic - Glass... Use the following settings:

Settings used:

  • Number of columns = 25
  • Symmetric = Checked
  • Glass curvature = 0
  • Edge curvature = 15
  • Grout width = 0
  • Group diffusion = 0

Press Ctrl+D to select none.

3. Go to Effects | Distortion Effects | Polar Coordinate... Use the following settings:

Settings used:

  • Rectangular to polar
  • Edge mode = Transparent

Now go to Effects | Distortion Effects | Ripple... Click the Reset to Default button, then change the Edge Mode to Transparent. Press OK. Now press Ctrl+Y 5 times to repeat the Ripple.

Press Ctrl+I to Flip the image.

4. (In this step, use the above image as a reference on how your image should look.)

Press S to activate the Selection Tool, then create a small circular selection in the center. Press delete to create a hole in the middle.

Use the Selection tool again, this time to create a rectangular selection of the bottom half. Press delete to get rid of the lower portion of the image. It should now look like the image above.

Press Ctrl+D to select none.

5. Press Ctrl+A, Ctrl+F, then Ctrl+Shift+F to select the fan.

Create a new layer. Press F to activate the Flood Fill Tool and fill the selection with a Pattern of your choice. I used Picnic. Set the Layer to Multiply with an Opacity of 75. Duplicate the layer. Set the layer to Dodge with an Opacity of 40.

To add the handle and strings, draw them in using the Pen Tool with the Width set to 3px and the Foreground Color set to a gradient of your choice. I used Metal Brass in the example. Hide the background layer, then merge the visible layers. Finally, if you want, add a Drop Shadow.

That's it! Now you can create an exotic geisha fan.
